When an EMS Agency in Allegany, Cattaraugus, or Chautauqua counties would like to add a new member to their roster or reinstate a former member, the following must be submitted to STEMS for each person on the Southern Tier Health Care System, Inc. LMS at: https://collabornation.net/login/STEMS
Additionally, if your agency is in Cattaraugus County and utilizes Dr. Walters as your Medical Director, please be sure to complete the checklist and required precepted calls form.
- Provider Privilege Application
- Skill sheet completed by an approved preceptor
- Patella Dislocation Certificate
- 2019 NYS BLS Adult and Pediatric Protocol Update through Vital Signs Academy
- Copies of certification and ancillary cards
- NYS EMT Card – All levels of care
- If you do not have a copy of your NYS EMT card, you can request a copy be sent to you via email.
- CPR – All levels of care
- ITLS – AEMT, Critical Care and Paramedic
- PALS – Critical Care and Paramedic
- ACLS – Critical Care and Paramedic
- NYS EMT Card – All levels of care
- Any other requirements set forth by your agency's Medical Director (be sure to consult with them on adding a new provider)
