When an EMS Agency in Allegany, Cattaraugus, or Chautauqua counties would like to add a new member to their roster, the following must be submitted to STEMS for each person:
Additionally, if your agency is in Cattaraugus County and utilizes Dr. Walters as your Medical Director, please be sure to complete the checklist and required precepted calls form.
- Provider Privilege Application
- Skill sheet completed by an approved preceptor
- Patella Dislocation Certificate through MLREMS
- 2019 NYS BLS Adult and Pediatric Protocol Update through Vital Signs Academy
- Copies of certification and ancillary cards
- Any other requirements set forth by your agency's Medical Director (be sure to consult with them on adding a new provider)
