Health Insurance Navigation

Get Insurance!
Southern Tier Health Care System (STHCS) has partnered with Healthy Community Alliance, Universal Primary Care and the New York State Office of Rural Health to provide free health-insurance enrollment services to residents of the Allegany, Cattaraugus and Chautauqua counties. This partnership is supported by the NY State of Health Marketplace which was created in response to the Affordable Care Act.

​Individuals, families and small businesses can explore their health insurance options, find out if they are eligible for tax credits and other financial assistance and enroll in a health plan that best fits their needs. A navigator can answer questions, identify required documentation, and assist with completing all components of the on-line application. 

 
Our goal is to help people access the health insurance they need to protect their health and wellness and that of their children and families. To schedule an appointment and learn more, please call Southern Tier Health Care System at 716-372-0614 ext. 201 or contact us. ​

Required documentation for each applicant:
  • Photo identification for primary applicant
  • Projected total gross income (last 4 pay stubs or letter of verification)
  • Tax Return Forms for previous year (if filed)
  • Social Security Number for applicant and EACH household member
  • Email Address
  • Insurance card, cost and term date if ending (if currently insured)
Submit Client Referral

If any of the below changes occur, please be sure to contact your navigator immediately as your coverage may be affected:
  • Change in income
  • Change in employment
  • Change in address
  • Planning to move out of state
  • Eligible for Medicare (on disability for more than two years)
  • Turning 65 years old
  • Marriage
  • Divorce
  • Pregnancy
  • Adoption
  • Losing health coverage
