Annual Skills due June 30, 2024
This is a reminder that the WREMAC annual didactic and clinical skills requirements are due to STEMS by June 30, 2024. Don't wait, turn in your annual skills today!

Each provider will need to submit their documentation on the Southern Tier Health Care System, Inc. LMS at: https://collabornation.net/login/STEMS.

For each individual the following documents must be submitted on the LMS:

• Skill sheet for each provider completed by a WREMAC approved preceptor
  • Please ensure that all information is completed on the skill sheet. This includes all expiration dates, email address and medical director. If any information is missing the provider will not be a candidate for online privileges until every item on the sheet is complete.
• Provider Privileges Application (form on LMS)
• Copies of valid certification cards
  • NYS EMT Card – All levels of care
    • If you do not have a copy of your NYS EMT card, you can request a copy be sent to you via email.
  • CPR – All levels of care
  • ITLS – AEMT, Critical Care and Paramedic
  • PALS – Critical Care and Paramedic
  • ACLS – Critical Care and Paramedic

Lastly, if you are an EMS captain, you can request administrative access that will give you the ability to view submissions of your providers and submit on their behalf if necessary. Please click here to request administrative access.
