Chautauqua County Office for the Aging

The Chautauqua County Office for the Aging provides a comprehensive array of services for older adults and caregivers located in Chautauqua County. Services and programs include:

Home Safety/Independence Programs:
Home Repairs and Access Program
  • ​​Provides assistance to adapt the home or bathroom for handicap access and to make critical home repairs where there is a safety concern.
Home and Community Care Services
  • Services may include help with housekeeping, personal care, home delivered meals, and personal emergency response systems. 
Telephone Reassurance
  • Automated phone calls to an elderly person daily. If the person cannot answer, an emergency contact or help is called immediately.
Personal Emergency Response System 
  • This is an easy to use medical alert service designed to reduce the risk of living alone. In the event or a fall or emergency, help is available at the push of a button.
Operation RUSH
  • Provides an identification bracelet or necklace to be worn by individuals who are prone to wandering.

Nutrition and Wellness Programs
:
  • Senior Wellness and Nutrition congregate meals are offered throughout the county, including locations in Brocton, Forestville, Fredonia, Findley Lake, Lakewood, Mayville, Sinclairville, and Westfield.
Meals on Wheels
  • A hot meal designed to meet one third of the recommended dietary needs is delivered to the home of those who are determined to be eligible to receive meals.

Caregiver Support Services:
  • The Office for the Aging offers in-home visits, counseling, support groups and respite care for caregivers of older adults. 

Social Adult Day Care Services:
  • Social model day care services are available for seniors who should not be left alone or who would benefit from supervision. Adult day care services are available in Irving, Dunkirk, three sites in Jamestown, and Westfield.

Other services:
  • Health Insurance Counseling and Assistance Program
  • NY Connects
  • Wellness/exercise programs
  • Case management and advocacy
  • Transportation services to medical appointments coordinated with the Chautauqua Area Regional Transportation System (CARTS)
  • Elder Abuse Prevention Program

NYS Cancer Services Program

​The CSP is a New York State cancer
screening program. We provide breast,
cervical and colon (also called colorectal)
cancer screenings and follow-up services
at no cost to women and men who:
• Live in New York State
• Do not have health insurance
• Have health insurance with a cost
share that makes the cost of screening
too high
• Meet the program rules for age
and income

Alzheimer's Association of WNY 

The Western New York Chapter of the Alzheimer's Association provides supports and information related to the specific challenges related to Alzheimer's disease and other dementias.

The Alzheimer's Association offers resources such as the Alzheimer's Navigator. It helps guide Caregivers to answers by creating a personalized action plan and  linking to information, support and local resources. They also offer a Caregiver Center that provides links to resources and training for caregivers.

NY Connects

NY Connects provides information about local services and supports for people of all ages or with any type of disability. NY Connects services can be accessed online at http://www.nyconnects.ny.gov/ or in person at the Chautauqua County Office for the Aging.

Total Senior Care

Total Senior Care is a Program of All Inclusive Care for the Elderly (PACE). Total Senior Care provides a wide array of services including: case management, physician, hospital, and nursing home care. The program is designed for adults 55 years old or older, who wish to live at home, but need assistance in order to remain independent.  

Total Senior Care also offers Daybreak. DayBreak is a social program for adults who may need assistance with activities of daily living. DayBreak provides activities and socialization in a supervised and safe setting. Services include an individualized plan of care, socialization, assistance with personal care, therapeutic recreational activities and nutritious meals and snacks. Individuals attend on a planned basis, ranging from one to five days per week, either full or half days, depending on individual or caregiver needs. Daybreak is offered in Olean, Gowanda or Wellsville.

Venture Forthe

Venture Forthe provides personal assistance services to help individuals in Chautauqua County with safely staying in their homes. Services include:
  • Home Care Support Services for individuals who need assistance in their homes, including monitoring home safety, help with meals, medications, transportation, bathing, light housekeeping and companionship.
  • Direct care services from Personal Care Aides who provide hands-on care and supervision.
