NYS ePCR and Paper PCR Portal

 The Bureau of Emergency Medical Services is pleased to announce three new opportunities for electronic submission of patient care to New York State at no cost for EMS agencies.  Two of the three exciting documentation methods will qualify EMS agencies to either initiate or continue participation in the NYS Continuing Medical Education (CME) Recertification Program. 

  1. BLS First Response Form *
  2. Ambulance and ALS First Response Form * 
  3. Paper PCR Portal **

​If your agency is looking to transition to use ePCR through a different vendor (i.e., emsCharts, ESO, HealthEMS, etc.) and your transition will not be completed by the March 31, 2021 deadline, you must select option 3 (Paper PCR Portal) while in the transition process. Once your transition to ePCR is complete, you would no longer be required to submit to the Paper PCR Portal. 

* Qualifies the EMS Agency to participate in the NYS Continuing Medical Education (CME) Recertification Program
** Does not qualify the EMS Agency to participate in the NYS Continuing Medical Education (CME) Recertification Program
 
All EMS agencies not currently documenting on an ePCR program must start the process for transition to one of these three options no later than February 28, 2021 and finish the transition no later than March 31, 2021.  EMS agencies encountering challenges meeting the March 31, 2021 deadline should work with their Regional EMS Program Agency and the Bureau of Emergency Medical Services to complete the transition; the Regional EMS Program Agency or the Bureau of Emergency Medical Services will assist in any way possible.  EMS Agencies may also select another NYS approved ePCR vendor for patient care documentation.
 
Continuing Medical Education (CME) Recertification Program
The 2019 release of the CME Program Manual states that “Agency participants must comply with Bureau of EMS Policy Statement 12-02 and complete a Patient Care Report (PCR) for every EMS response.  Agencies participating in the CME recertification program will be required to submit PCRs to the department electronically on or before January 1, 2022.”
 
Policy 12-02 Pre-Hospital Care Reports
EMS agencies must complete a PCR/ePCR each time the EMS agency is dispatched for any type of response; including, but not limited to:
  • Patients transported to any location
  • Patients who refuse care and/or transport
  • Patients treated by one agency and transported by another
  • Calls where no patient contact is made, such as:
    • Calls cancelled before reaching the scene
    • Calls where no patient is located
    • When dispatched for a stand-by
    • Events
 
Policy 12-03 Electronic PCR Data Submission
EMS agencies must adhere to all of the following:
  • Be in compliance with all applicable sections of Article 30 and Part 800
  • Be currently submitting paper PCRs to the Regional Program Agency on a routine and on-going basis
  • Contact the Department, in writing, to determine electronic reporting requirements and request approval for electronic submission (DOH 5136, which is submitted through your regional program agency), and request approval for electronic submission
  • If the software being considered for purchase is not currently mapped and submitting to the NY state data repository, testing of the data compliance must occur to insure proper format and electronic transmission of the satisfaction of the Department and the Regional Program Agency
  • Submit PCR data to the Department in the specified data file format at predetermined and scheduled intervals
  • Receive endorsement from the appropriate Regional Emergency Medical Services Council(s) (REMSCO) and Regional Emergency Medical Advisory Committee(s) (REMAC) in writing
  • All EMS services must submit the standard NYS data file to the Regional Program Agency in a compatible format on a regular and routine schedule determined by the program agency.
  • Apply for, and receive, an account with the Department’s Health Commerce System (HCS).  This may be done with assistance from the Regional Program Agency.
  • If any changes or interruptions are made to the electronic patient record system that may affect data submission, the EMS service must notify the Department, in writing, ten (10) business days in advance of implementation.  It is the Department’s expectation that once a service converts to an electronic data collection (ePCR) system, that service will maintain the electronic system and NOT revert back to a paper-based system.

OPTION #1:  BLS First Response Documentation Standard and BLS FR ePCR Entry Form

A team of BLS FR Agency Leadership, Regional Program Agencies and the Bureau have collaborated to produce a documentation standard built around the time factors, response environment and expected limited patient contact time that allows a significantly reduced number of required fields, allows for electronic participation in regional QI programs, and eligibility in the Continuing Medical Education (CME) Recertification Program.
 
The BLS FR ePCR Entry Form on ImageTrend is available at no expense for any BLS FR EMS agency in New York State.  If the EMS Agency wishes to include additional services from ImageTrend (CAD Integration, Automated Billing Export, NFIRS Reporting or Elite Field (the ability to enter into the program without a wireless connection) there is an additional separate expense contracted between the EMS Agency and ImageTrend directly.
 
If your EMS Agency is selecting ImageTrend, the software is recommended to be used on a Microsoft Surface Pro 3 or greater, Motion R12, Panasonic Toughpad FZ-G1, Panasonic Toughpad CF-20, iPad Air (and Air2) – 9.7”, iPad Mini (and Mini HD, Mini 2, Mini 3) – 7.9”, Galaxy Tab Pro – 8.4” or Nexus 10.  The software is supported on Safari, Chrome, Chrome Mobile or Firefox browsers.
 
EMS agencies wishing to start documenting electronically or transition to the free BLS First Response form, should follow the directions on the DOH 5136 form and submit the form to the Regional Program Agency following the timeframes indicated on the template.

OPTION #2:  Ambulance & Advanced Life Support ePCR Entry Form

A team of ALS and BLS Agency Leadership, Regional Program Agencies and the Bureau have collaborated to produce a documentation standard as a response to feedback received from EMS Providers, EMS Agency Leadership, Regional Program Agencies, Medical Directors and ePCR Software Vendors.  The documentation standard has been developed to be situationally relevant, encourage comprehensive professional pre-hospital care reports and facilitate the utilization of pertinent negatives where fitting and appropriate. 
 
The Ambulance and Advanced Life Support ePCR Entry Form on ImageTrend is available at no expense for any Ambulance or Advanced Life Support EMS Agency.  If the qualifying EMS Agency wishes to include additional services from ImageTrend (CAD Integration, Automated Billing Export, NFIRS Reporting or Elite Field (the ability to enter into the program without a wireless connection)) or ECG monitor importing there is an additional separate expense contracted between the EMS Agency and ImageTrend directly.  
 
The implementation of the Ambulance and Advanced Life Support ePCR Entry Form on ImageTrend by an EMS Agency qualifies the EMS Agency to participate in the Continuing Medical Education (CME) Recertification Program.

OPTION #3: Paper PCR Portal

The Bureau is transitioning the submission of the yellow copy of the Version 5 Paper PCR from the Regional Program Agency to a direct submission by the EMS Agency through an online portal immediately after the response is completed. 
 
The Paper PCR Portal has been created to require no more than 40 fields to be entered and allow concise documentation of procedures performed or medications administered.  The Paper PCR Portal allows for ease in population in regional Quality Improvement programs as well. 
 
If your EMS Agency is selecting ImageTrend, the software is recommended to be used on a Microsoft Surface Pro 3 or greater, Motion R12, Panasonic Toughpad FZ-G1, Panasonic Toughpad CF-20, iPad Air (and Air2) – 9.7”, iPad Mini (and Mini HD, Mini 2, Mini 3) – 7.9”, Galaxy Tab Pro – 8.4” or Nexus 10.  The software is supported on Safari, Chrome, Chrome Mobile or Firefox browsers.
 
Submitting Paper PCRs through the Paper PCR Portal does not qualify an EMS Agency to participate in the Continuing Medical Education (CME) Recertification Program.
 
Regional Program Agencies will no longer provide paper PCRs after 03/31/2021.  EMS Agencies seeking 1:1 replacement paper PCRs must request from the Data and Informatics Unit at https://apps.health.ny.gov/pubpal/builder/survey/ems-survey-17-1 .
 
EMS agencies wishing to submit their paper PCRs through the Paper PCR Portal should follow the directions on the DOH 5136 form and submit the form to the Regional Program Agency following the timeframes indicated on the template.

Enrollment in One of These Programs

EMS Agencies must enroll in one of these opportunities, if your EMS Agency is not already documenting electronically, by contacting their Regional Program Agency to arrange for a demonstration or to discuss in greater detail.  When the EMS Agency makes a final decision on either the BLS FR ePCR Entry Form, the Ambulance and Advanced Life Support First Response ePCR Entry Form or the Paper PCR Portal, a completed DOH 5136 (see samples earlier in this email) must be submitted to the Regional Program Agency for endorsement.  The standard timeline from submission to approval should be completed within three (3) weeks for processing.

Vaccination Training

Overview

Become a vaccinator!
New York State is encouraging eligible New Yorkers to take the required trainings to become vaccinators and support the State's COVID-19 vaccination program.
Governor Cuomo signed an Executive Order expanding the eligible pool of trainees who can administer vaccinations at a "POD site" (flexible vaccination sites that can be set up in any community) pursuant to a non-patient specific order provided they first receive training. These include:
  • Licensed practical nurses (LPNs)
  • Pharmacists who are not certified to administer immunizations by SED
  • Midwives who are not certified to administer immunizations by SED
  • Dentists
  • Dental hygienists*
  • Podiatrists
  • Emergency Medical Technicians
  • Advanced Emergency Medical Technicians
  • Students in eligible education programs (medicine, nursing, PA, pharmacy, dentistry, podiatry, and midwifery)
*Must have been issued a dental hygiene restricted local infiltration anesthesia/nitrous oxide analgesia certificate
All POD sites must have oversight from or be approved by NYSDOH or the applicable Local Health Department, and must have medical oversight by a licensed physician, licensed physician assistant (PA), or certified nurse practitioner (NP). These individuals must have a current certification in CPR.

Training

Required Training Under the Executive Order1. Complete the online NYSDOH COVID-19 Vaccine Training for Vaccinators modules.There are four training modules:
  • Background on COVID-19 Vaccines:
  • COVID-19 Vaccine Considerations, Patient Screening, Education, and Consent
  • COVID-19 Vaccine Administration, Anaphylaxis, and Post-Vaccination Information:
  • COVID-19 Vaccine Shipping, Storage, and Handling

2. Complete the in-person clinical vaccine administration training with assessment of skills and competencies signed by a supervisor or faculty member using the Immunization Action Coalition Skills Checklist for Vaccine Administration with all areas identified as meeting or exceeding required skills and competencies for vaccine administration
  • For students, the checklist must be signed by the educational institution in which they are enrolled. In addition, a designee from each institution must complete an attestation for any student vaccinator, attesting that such student vaccinator has completed all required trainings and possesses the requisite immunization administration skills to administer influenza and COVID-19 vaccinations in a POD site overseen or approved by the NYSDOH or LHD
  • For all other new vaccinators, the checklist may be signed by:
    • An MD, PA, NP, or RN OR
    • A certified EMS provider designated by the POD’s highest medical authority who has reviewed and signed off on the skills of the evaluator. Preferred EMS evaluators include Certified Lab Instructors, Certified Instructor Coordinator, Field Training Officers, EMS Training Coordinators, Agency leadership.

3. Have a current certificate in basic CPR, which at a minimum must include certification by an online program that has received accreditation from the American Nurses Credentialing Center, the Accreditation Council for Pharmacy Education (ACPE), or the Accreditation Council for Continuing Medical Education

4. (For students): Have completed, at a minimum, at least 1 year of clinical experience.

How to Complete the Required NYSDOH COVID-19 Vaccine Online Trainings for Vaccinators1. For non EMS providers, you must sign in or register on the New York State Department of Health's Learning Management System (LMS).
For EMS providers, go to www.VitalSignsAcademy.com and click on the top right hand side of the page “Login/Register”.

2. You must complete four training modules in your system.
To access the direct modules in the LMS system:
  • Module 1 - Background on COVID-19 Vaccines

We wanted to make sure this document got out as quickly as possible and recognize it will generate additional questions.  
A Statewide EMS Leadership call will be scheduled the near future to further discuss the document and answer questions.

ACT NOW - EMS Needs to Be Tier 1a Vaccinated

Within the last 24 hours, despite national recommendations and previous planning considerations, NYS is NO LONGER including EMS personnel in Tier 1a (First Distribution) availability for COVID Vaccine.  Given our unique work environment – operating in a confined space for extended periods of time with COVID patients – there are no words for my frustration. 

Specifically “Following the vaccination of 'high risk' health care workers, the priority will shift to all long-term and congregate care residents and staff, EMS and other health care workers.”  How EMS is not considered “High Risk” is beyond my comprehension.  https://www.governor.ny.gov/news/governor-cuomo-updates-new-yorkers-states-vaccination-distribution-plan 

Please consider communicating to elected officials, specifically the Governor and Commissioner of Health, as well as any other elected leaders at the state, County, and even federal level, that EMS must be included alongside our high risk hospital care providers else our already crumbling system will collapse.

Attached is a toolkit from the AAA and a draft letter that you may edit and communicate to elected officials.  Email, call, smoke signal.  Get the word out.

The need is now, this is moving fast, do not wait.  Vaccine will be here in days and I fear we may have already missed the window to advocate for our own lives.  Communicate to elected media, leverage media connections and use social media.  The need is now although I fear too late as this rug was pulled out from under us.

New PPE Guidance

With the recent increase in COVID-19 cases in the county and region we have updated our PPE recommendations for EMS providers.  We want to assure providers that this is not being done due to any exposure or incident with our providers. Our guidelines from the beginning have held up, kept our providers from getting infected, and are still inline with CDC recommendations.
That being said, out of an abundance of caution we are increasing some of our PPE recommendations due to the high community transmission rate we are seeing to further protect our providers.  In recent discussions with many of you, it appears many agencies are already utilizing an approach similar to these changes.  These are also similar to what our hospitals are employing.
Attached is the updated guidance. There are a few changes I would like to highlight.    
  • All providers should continue to wear a surgical mask, eye protection, and gloves for all routine calls
  • All patents should be screened for symptoms of COVID-19 regardless of call nature.  The symptoms list and risk factors have been expanded and updated according to current CDC facility screening questions.
  • If a patient screens positive by answering “YES” to any screening question, then they should be considered a possible infectious disease (PIDS) case.  
    • Providers should then don an N95 instead of just a surgical mask.  (Even if no aerosol procedures).
    • Face shields are recommended over safety goggles in this instance.
    • For aerosol generating procedures, wearing of a gown should be added to the above PPE.

Dear Future EMT Original Participant

EMT Program registration is PENDING DOH APPROVAL.  All registrations will be processed when approval comes in.  You will receive acceptance notification when all information is processed. PLEASE COMPLETE ATTACHED APPLICATION AND PAPERWORK.  This will make the process easier when approval is received.

  • EMT-B REFRESHER application

Due to the COVID 19 pandemic, all forms and responses for the EMT program will be handled electronically.

Once the application is completed and emailed back to me, you will have until September 10th to complete the items on the checklist.

The Student Data form and the Certificate of Residence for Chautauqua and Cattaraugus Counties can now be completed online.

The rest of the information can be sent via email to the parties listed on the form. PLEASE NOTE IN IN YOUR EMAIL THAT YOU ARE AN EMT STUDENT.

Emergency Vehicle Travel Advisory - Front St, City of Olean

​Please see the attached Travel Advisory from Chief Richardson, Olean City Fire Department. Road Construction on Front Street in the City is in progress and is expected to hinder emergency vehicle traffic that is inbound to Olean General or Route 16 from Route 417, East of the City of Olean.   
 
Emergency vehicles traveling on Route 417 west bound  (from East of the City) towards Olean General and/or Route 16  are encouraged to follow the "Emergency Vehicle Route" that is outlined on the attached map. Additionally "Emergency Vehicle Route" signs have been installed along the alternate route.
 
Construction is expected to be in progress for at least the next 30 days.
