NYS 1115 Waiver

New York is launching a new program under its Medicaid Section 1115 Demonstration Waiver to bring together healthcare and social services with dedicated funding for the first time. The Western New York Integrated Care Collaborative (WNYICC) has been chosen as one of nine groups across the state to share $500 million over the next three years to create a Social Care Network (SCN) program. These SCNs will focus on meeting essential needs like nutrition, housing support, transportation, and case management for eligible Medicaid members. This effort aims to reduce health disparities and improve overall community health. STHCS is excited to join this effort and work with WNYICC on this project. We look forward to collaborating to make a lasting, positive impact on the health and well-being of our community. ​
