Child & Family Health Plus
Southern Tier Health Care System, Inc. (STHCS) is the lead agency for facilitated
enrollment of Child and Family Health Plus in Cattaraugus County, eastern Chautauqua
County and western Allegany County (Allegany and Chautauqua counties only started in
2007).  The organization has a referral system set up with local health care and social service
agencies.

Child Health Plus and Family Health Plus are health insurance programs aimed at low- to
moderate-income children and families in New York State.  Our facilitated enrollers assist
families with completing the application, collect necessary documentation and submit the
completed application to the appropriate agency.  Eligibility is based on income/resource
guidelines determined by the New York State Department of Health.

Medicaid) or Child Health Plus B. Both Child Health Plus A and B are available through
dozens of providers throughout the state. Enrolling in Child Health Plus is easy.

  • With both Child Health Plus A and B, most of your child's health needs will be covered.
    In addition, Child Health Plus A covers transportation to doctor's appointments and
    orthodontia treatment. Child Health Plus A also has an added guarantee that provides
    for all medically necessary treatment.
What is Family Health Plus?

Family Health Plus is a public health insurance
program available to adults between the ages of 19
and 64 who do not have health insurance either on
their own or through their employers but have
incomes too high to qualify for Medicaid. Family
Health Plus is available to single adults, couples
without children, and parents with limited income who
are residents of New York State and are United
States citizens or fall under one of many immigration
categories.

Who can join?

Family Health Plus is available to single adults,
couples without children, and parents with limited
income, who are between the ages of 19 and 64, are
residents of New York State and United States
citizens or fall under one of many immigration
categories. If you have health insurance - either on
your own or through your employer, or are eligible
school district benefit plan - you are not eligible to enroll.

There are limits on the amount of assets or resources that you can have and still be eligible
bonds, mutual funds, certificate of deposit, money market accounts, 401K plans, trust funds,
the cash value of life insurance, motor vehicles, or property that someone owns. The value of
your home will not be counted. The amount of income and assets/resources you and your
family can have and still be eligible for Family Health Plus depends upon how many people
are in your family.

How to apply?

To apply for Child or Family Health Plus, you will need to have a personal interview with a
STHCS facilitated enroller where you will complete an application, provide proof of certain
information and select a health plan.

Our Enrollment Specialists are available Monday through Friday from 8:00 a.m. until
5:00 p.m.  Although walk-ins are welcome as long as we have an enroller available, it is best
to call ahead and make an appointment.  We also travel to several locations in our catchment
